The exact answers depend on which university and which department, but typically you should both apply through the formal avenue and try to find suitable research supervisors at the same time. 


In areas with high demands (natural sciences, engineering, math), top US universities give full scholarships which include tuition and stipend. However, they do not always advertise this. 


Your questions are too vague, you should be more specific (consider instead "I am interested in studying X in the Y department at ZZZ universities, I received by A degree in B from C, and my grades are D, my publications are E").
